Registration
Any person living within the agreed practice boundary may register as a patient with the practice. When registering, please bring your medical card to the practice along with those of any other persons registering with you. You will be asked to complete a registration form for each person registering with the practice. Proof of identity will also be required – a passport or photo driving licence is preferable, but a birth certificate is acceptable.
When registering, all new patients will be asked to complete a new patient questionnaire. This provides us with basic information about your health and allows us to provide medical care while your medical records are transferred from your previous practice. You will also be asked to make an appointment to come for a new patient check.
You register with the practice not one doctor. Your medical card will be issued in the name of one of the doctors in the practice. However, you can at any time express a preference for a particular practitioner, for either all of your medical needs, or on a case-by-case basis. This preference can be recorded in your medical notes by request. The practice will always try to comply with your requests but this may not always be possible.
If you move out-with the practice boundary, you will need to register with another practice as soon as you move.
